Registered Nurse / Licensed Practical Nurse

Nurses who are licensed in another state and have never held a Maryland license may apply for licensure by endorsement.
NOTE - If you DO NOT currently reside in Maryland, please send an e-mail to mdbon_nets@dhmh.state.md.us with your name and current mailing address to receive the required Background Check forms prior applying on-line. Maryland residents ONLY may use this form [PDF] to complete the Background Check.
