On-Line Certification Applications

​Effective January 27, 2012:  Please be advised that the disclosure of your Social Security Number is mandatory in order to process your application.
 
Any license or certificate application(s) received at the Maryland Board of Nursing without a Social Security Number (SSN) WILL NOT be processed.  Applications without a SSN are incomplete.
 
The Board is required by federal and Maryland law to collect this information for the following purposes:
  • Verification of identity with respect to final adverse actions related to your license or certificate (42 U.S.C. § 1320a-7e(b))
  • Administration of the Child Support Enforcement Program (Md. Family Law Code Ann., § 10-119.3)
  • Identification by the Maryland Department of Assessments and Taxation of new businesses in Maryland (Md. Health Occ. Code Ann., § 1-210)
    Primary source verification of certification status is found in the "Look Up A License" feature of this website. This secure program is updated daily. Paper licenses are not secure and are only accurate at the time they are printed. Paper licenses and certificates are easy to forge and are used by impostors for fraud and identity theft; these are serious issues that threaten our mission of public protection. In the rare event an employer will not accept the Look Up A License verification, you may request a paper verification form.
 
Passport-style Photograph Required

A properly formatted passport-style photo is required to be submitted with all initial licensure and certification applications. Please click here for examples of passport-style photos. If you have a digital photo close to the example, you can use this on-line tool to upload and crop your photo to meet the passport-style standard. (Adobe Flash Player 10 or later required)
 
NOTE: ANY PHOTO SUBMITTED NOT MEETING THE PASSPORT-STYLE FORMATTING STANDARD WILL DELAY THE PROCESSING OF YOUR INITIAL APPLICATION!
